At Our Community Place, we foster a secure and nurturing environment, collaboratively working hand in hand with community members to empower them rather than working for them. Our unique approach helps individuals reclaim a sense of empowerment and control. Many of our community members have experienced distressing or traumatic incidents that influence their daily behaviors and life decisions. Our care model is rooted in trauma-informed practices and strength-based approaches.OCP offers a comprehensive case management program, from supporting those experiencing homelessness to facilitating their transition to permanent supportive housing. We employ effective strategies from the Housing First approach and Strength-Based Supportive Case Management models. Each member of our case management program sets personalized goals, developing pathways and skills to achieve them with the ultimate aim of attaining permanent housing. Through weekly individual and group meetings, community members are encouraged and empowered to take constructive steps away from destructive addictions and unhealthy behaviors leading to homelessness. Our Housed Support Group collaborates with community members to acquire essential skills, including budgeting, meal planning, grocery shopping, household maintenance, strengthening social bonds, job skills, and cultivating healthy relationships.Join us in making a difference.
