We work with the fastest growing and most under-served segment of our unhoused population in San Diego, older adults who have lost their housing and can be found sleeping on our streets or in their cars. We find many of our participants outside of hospitals, so they come from all over the county.Most of our participants are ‘older orphans’. In many ways we take the role of a family member or close friend and do whatever it takes to help people stabilize their health and find housing. This kind of engagement is especially important for older adults who often struggle with systemic barriers to care that rely on largely virtual environments. Once they start to experience cognitive impairment these barriers become insurmountable without help. They need personalized intensive support, the kind that very few organizations are able to provide, the kind of care that H4H specializes in. This goes beyond the 'navigator' model. We take a holistic approach which honors the fact that everything in a person's life is interconnected. We support their experience of autonomy, competence,  and dignity, restoring their connection to a hopeful and thriving future.
